The Pentagon issued detailed clinical guidelines Wednesday for a mandatory testosterone deficiency screening program covering all active-duty and reserve service members age 30 and older — implementing, with immediate effect, a policy Defense Secretary Pete Hegseth announced in July and framed as essential to maintaining the military's fighting capacity. The guidance was released by the Defense Health Agency and applies to the entire active and reserve force. Medical experts, meanwhile, have raised sustained questions about whether universal testosterone screening has a scientific basis as a readiness tool.

The guidelines establish standardized protocols for both screening and treatment. Male service members age 30 and older will receive mandatory annual testosterone blood tests as part of their regular health assessments. Those under 30 will be offered optional screening or tested if a clinician identifies clinical warning signs. Service members diagnosed with hypogonadism — clinically low testosterone — will receive follow-up screenings and be offered testosterone replacement therapy. Hegseth said in July that accepting treatment remains the service member's choice: "It's entirely your choice to receive testosterone replacement therapy." The guidance also specifies treatment pathways for different levels of testosterone deficiency and outlines protocols for monitoring during treatment.

The guidance for female service members takes a different form. Rather than mandating routine testosterone blood tests, the guidelines call for screening for fatigue and disrupted menstrual cycles — symptoms the document associates with hormonal dysregulation and conditions including low energy availability and relative energy deficiency in sport. The document specifies that off-label testosterone prescribing for women is appropriate primarily for postmenopausal women with unusually low sexual desire.

Hegseth, who has publicly promoted testosterone as a marker of physical and cognitive performance, announced the mandate on July 15 in a social media video titled "The High-T Department of War." He framed the policy in terms of biological optimization rather than medical treatment, describing low testosterone as a readiness liability that the military had failed to address. "This initiative is not about artificial enhancement," he said. "It's about restoring and optimizing your natural capabilities, protecting your longevity and ensuring you have the biological foundation required to sustain the fight." The Defense Health Agency's guidance echoes that framing, describing testosterone as "a critical biological marker for Warfighter readiness, directly impacting physical strength, bone density, aerobic fitness, depressive symptoms, and cardiovascular health."

The medical community's response has been skeptical. Physicians and military health researchers have questioned whether universal testosterone screening among active-duty personnel — regardless of symptoms — would produce meaningful improvements in combat readiness. The concern is twofold: first, that identifying subclinical or borderline testosterone levels in asymptomatic service members may not warrant treatment, and second, that testosterone replacement therapy in otherwise healthy individuals carries its own risks, including cardiovascular effects, polycythemia, and hormonal suppression of natural testosterone production. Several military physicians, speaking to trade publications since Hegseth's July announcement, said the evidence base for population-level testosterone screening in a healthy military cohort is not comparable to evidence supporting screening in older civilian populations where clinical hypogonadism is more prevalent.

The Food and Drug Administration is separately scheduled to convene a panel of outside experts in mid-September to discuss the medical use of testosterone — a meeting whose timing, coming weeks after the Pentagon's mandate, adds a layer of institutional tension to the rollout. The FDA has not announced the specific scope of the September discussion, but it is understood to cover prescribing practices and off-label use, areas directly implicated by the Pentagon's new guidance.

The policy arrives in the context of a broader set of changes Hegseth has made to military personnel standards and culture since taking office. Under his leadership, the Pentagon has moved aggressively to remove diversity, equity and inclusion programs, reinstate service members discharged under COVID-19 vaccine mandates, and restructure promotion criteria and training programs to emphasize what administration officials describe as combat lethality. The testosterone screening mandate fits within that agenda — it is framed as a performance optimization program rather than a medical intervention, and it positions hormonal status as a military readiness variable that the armed forces should actively manage rather than leave to individual initiative.

Critics of the policy, including some congressional Democrats, have characterized the mandate as a culture-war gesture dressed in medical language, arguing that it prioritizes ideological signaling over evidence-based health policy. Supporters, including several retired special operations officers who have appeared on media platforms associated with the administration, argue that testosterone optimization is already standard practice in elite athletic and special operations communities and that extending it across the force is a logical step.

Neither argument settles the underlying empirical question, which the FDA's September panel and any peer-reviewed research emerging from the program's implementation may eventually begin to address: whether mandatory screening of the full active-duty force for testosterone deficiency produces measurable improvements in military readiness, or whether it produces primarily treatment costs, potential side effects, and a large number of clinical encounters for service members who would otherwise be asymptomatic.

The guidelines are in effect as of Wednesday. The Defense Health Agency has not specified a timeline for full implementation across all branches, or for the first required cycle of testing to be completed.
